Form IM-4, Important Information About Your Medical Assistance Benefits brochure is revised to remove Division of Family Services as well as obsolete eligibility information.
An initial distribution of this brochure is being sent to each county office. Upon receipt, destroy/recycle any revisions previous to 09/06. The brochure may be ordered from the warehouse using normal ordering procedures. It will be available on the intranet soon.
NECESSARY ACTION:
- Review this memorandum with all appropriate staff.
Recycle/destroy all versions of the brochure with a revision date prior to 09/06.
