DISCUSSION:
The Division of Medical Services (DMS) has announced a new voluntary program offered to Medicaid fee-for-service clients identified as having a chronic illness. The Chronic Care Improvement Program (CCIP) is an enhanced primary care case management program that incorporates the principles of disease management, care coordination, and case management to serve patients identified through a risk assessment and disease stratification model.
CCIP increases involvement of a central primary health care provider, empowers patients to perform self-management of their health status and utilizes existing community resources and health infrastructures through the coordination of care.
The goals of this program are to:
- Ensure the quality of care and health outcomes for CCIP participating members;
- Reduce inpatient hospitalizations;
- Lower total costs by educating CCIP members;
- Achieve provider and facility satisfaction with health management;
- Focus initially on Diabetes, Asthma, Gastroesophageal Reflux Disease (GERD), Chronic Obstructive Pulmonary Disease (COPD); Sickle Cell Disease, and Cardiovascular Disease.
APS Healthcare has been selected to administer the program. Clients are screened by APS and if they meet certain criteria are invited to participate. As CCIP is a voluntary program, clients have the option of participating when invited to enroll and can opt out of the program at any time. APS has recently begun sending enrollment packets to selected clients informing them of the new enhanced primary care case management program. In addition to mailings to identified CCIP members, APS also contacts identified CCIP members by telephone. DMS has also posted a provider Bulletin on the DMS Website at http://dss.missouri.gov/dms/providers/pdf/bulletin29-22_2007jan16.pdf to educate providers on how the program works for them.
