SUBJECT: 'THE WORK NUMBER' ACCESS FORM

DISCUSSION:

“The Work Number” Registration Worksheet to request access to “The Work Number” has been revised and renamed “The Work Number” Access Form. A copy is attached to this memo. Please discontinue using all previous versions and use the attached form until it can be added to the Intranet. Instructions for completing the form are also attached.

Effective immediately, this form must be completed electronically, in Microsoft Word. The form must then be printed and signed by the user and his/her supervisor. After completing “The Work Number” Access Form, fax it to Central Office, IM Program and Policy Section at 573/526-5592. The original form will be kept in the employee's personnel file at the local county office. Please note that handwritten requests will no longer be processed.

Once you have submitted your request to Central Office, IM Program and Policy Section, it will take approximately 2 weeks to be processed. At that time you may try to use “The Work Number” following the guidelines available on the FSD Intranet. If you are still unable to access “The Work Number”, please contact Central Office, IM Program and Policy Section at 573/526-4836.

Change in Authorized User Number Format:

This memo introduces a new format for the authorized user number. Currently, the user's social security number is used as the authorized user number. Effective with this memo, the user's five (5) digit worker ID number and the last four (4) digits of his/her social security number will be used as the authorized user number. This format will be utilized when registering new users from this point forward. Active users requesting changes to their information will continue to use their current authorized user number.

Changes to Registered User's Information:

A form must be submitted requesting a change to the user's information in the following instances:

Changes will take approximately 2 weeks to be processed. Users can continue accessing “The Work Number” using their old information until the change has been processed. Once the change has been processed, the user will receive an error message when trying to access “The Work Number” with the old information. The user must then attempt to use his/her new information to access “The Work Number”. If the user is still unable to access “The Work Number”, please contact Central Office, IM Program and Policy Section at 573/526-4836.

Deleting Access:

A form must also be submitted by the user's supervisor (user's signature not required in this instance) to delete the user's information in situations including, but not limited to:

Problems with “The Work Number”:

If a user is unable to access (log into) “The Work Number”, the user must contact Central Office, IM Program and Policy Section at 573/526-4836.

If a user has problems/concerns with the employment/income information provided by “The Work Number”, the user must call “The Work Number” Customer Service Center at 1-800-996-7566. Representatives are available Monday through Friday, 7:00 am till 8:00 pm, CST.
