DISCUSSION:
At the request of field staff and the Hearings Unit, Form IM-87, Application for State Hearing and instructions are revised. In addition to Program names, these changes are made to clarify the EU and the person requesting the hearing:
- Case name is added to the top section;
- Name of Person requesting Hearing is changed to read as “Name of the Household Member Requesting this Hearing”;
- Mailing Address is changed to read as “Household Mailing Address”; and
- Claimant's Signature is changed to clarify that the claimant or his/her representative can sign the IM-87.
To access the PDF version and/or instructions,
- go to Family Support Division (FSD) Intranet site,
- select Food Stamps,
- under Forms select Forms Manual from the drop-down list,
- select Volume 2, and
- select IM-87 or Instructions.
Use the current form IM-87 to depletion. Order the IM-87 from the warehouse using normal ordering procedures. The revised IM-87 (05/08) will soon be available in Word version.
