DISCUSSION:
Effective Tuesday, September 8, 2009, the Missouri Employment and Training (METP) Referral and Response form has been added to FAMIS. The FA-333 Referral and Response form is used to re-refer Food Stamp EU members to the Division of Workforce Development (DWD) to cure or end a Food Stamp sanction and replaces the IM-311 Referral and Response form.
Generating the Form
The FA-333 Referral and Response form can only be generated for individuals in application or active status. The procedure for generating an FA-333 Referral and Response form differs for individuals who are:
- coded 22 MNC and
- those who are already coded 25 MNC on the Employment Assessment (FMMS or EMPLOY) screen.
The procedure for each code is outlined below
The message “Invalid EU member status” displays on the Employment Assessment (FMMS or EMPLOY) and the Verification Forms List (FM4B or VERFORM) screens when attempting to create an FA-333 Referral and Response form for an EU member who is not in application or active status.
Individual Coded 22 MNC
To generate the form from the Employment Assessment (FMMS or employ) screen in FAMIS:
- Change the METP code from 22 MNC to 25 MNC. The FA-333 Referral and Response form is automatically generated.
- The message “Form generated successfully—use DOCQUE to edit/print the form” displays at the bottom of the screen unless the last person whose code is added is not the sanctioned individual.
If the person whose code is changed is not the sanctioned individual, the form is generated and the message “Explore sanction and update if needed” displays.
The FA-333 Referral and Response form is then ready to edit. Refer to the Editing FA-333 section below.
Individual Coded 25 MNC
When an EU member who is already coded 25 MNC indicates s/he wishes to comply with DWD to end their MTP sanction, enter a comment regarding the re-referral on the Employment Assessment (FMMS or EMPLOY) screen.
To generate the form from the Verification Forms List (FM4B or VERFORM) screen in FAMIS:
- Enter the DCN for the sanctioned individual in the DCN field.
- Scroll to the FA-333 Referral and Response form and select it.
- Press F23=GENFORM to create the form. The message “Form generated successfully—use DOCQUE to edit/print the form” displays.
- Press F17=DOCQUE to go to the Document Queue (FMVM) screen.
Editing FA-333
The FA-333 Referral and Response form must be edited from the Document Queue (FMVM or DOCQUE) screen. When going to Document Queue from the Employment Assessment (FMMS or EMPLOY) screen, enter the head of the EU’s DCN in the DCN field and press enter. The FA-333 displays in the lower section of the screen.
- Select the FA-333 and press F17=EDITDOC. The Select Field Edit (FMVT) screen displays. Type U on the command line.
- Select the Agency Address field (mandatory) and type the name of the DWD office serving your county or the office to which the individual is being re-referred.
- Select the Agency Address field and type the address for the DWD office.
- Select the Contact Number field and enter the telephone number for the EU member being referred, if available.
- Select the Text field to enter any comments you wish to share with DWD.
- Select the Fax Number field to enter the FSD office FAX number and press enter.
- Press F21=RETURN. The Document Queue (FMVM or DOCQUE) screen displays.
- Tab to the FA-333 and type P on the Com line. Press F23=PRINT to print the form.
The Individual, address, city, state, zip, DCN, Social Security number and date of birth fields are automatically populated in the Individual Referred section. The eligibility specialist, telephone number, referring address, city, state, and zip code fields are also automatically populated in the Comments Section.
The referral date is populated with the date the FA-333 is generated. The FA-333 Referral and Response form should be printed the same day it is created. If the FA-333 is not printed on the same day it is created, delete the form and generate a new form on the Verification Forms List (FM4B or VERFORM) screen so the correct referral date is printed on the form.
Once the form is printed, check the appropriate box in the "Referred For" section. Make a copy of the completed form for the case record. Give the completed form to the participant to take with them to DWD.
NECESSARY ACTION:
- Review this memorandum with all appropriate staff.
- Recycle the current supply of IM-311 Referral and Response forms.
