Due to the Program Integrity Review completed by Food and Nutrition Services (FNS) the Advance Notice of Your Administrative Disqualification Hearing (IM-160) form and Waiver Of Administrative Disqualification Hearing And Consent to Disqualification (IM-161) form have been revised.
The updates include minor formatting changes and adding the following participant information:
