DISCUSSION:
The purpose of this memorandum is to introduce the Spanish version of the IM-1SSL (Spanish) and the Spanish version of the IM-1ADP (Spanish).
Provide the IM-1SSL (Spanish) and IM-1ADP (Spanish) forms to participants who request the Spanish version Family MO HealthNet applications. Updated PDF versions of the forms are available in the Forms Manual on the intranet

NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Provide the IM-1SSL (Spanish) to applicants who request a Spanish version of the IM-1SSL.
- Provide the IM-1ADP (Spanish) to applicants who have more than two persons in the assistance group who request a Spanish version of the IM-1SSL.
