- SUBJECT:
- IM AUTHORIZED REPRESENTATIVE REVOCATION (IM-6ARR)
- FORMS MANUAL REVISION #27
DISCUSSION:
The purpose of this memorandum is to explain changes to the IM Authorized Representative Revocation (IM-6ARR) form.
On the previous version of the form, instructions in the section “Final Disposition” explained actions an eligibility specialist must take in the Family Assistance Management Information System (FAMIS). With the implementation of the Missouri Eligibility Determination Enrollment System (MEDES), these instructions are removed from the IM-6ARR.
If a participant provides a signed an IM-6ARR, the eligibility specialist must end the authorized representative in FAMIS or MEDES.
To end an authorized representative in FAMIS, an eligibility specialist must go to the Representative List (AUTHREP/FMJ1) screen. Enter the current date on the end date line in the Representative Details (FMJG) screen. The eligibility specialist must make a comment in the Representative List (AUTHREP/FMJ1) and in the Eligibility Unit Member Role (EUMEMROL/FM3Z) screens regarding the receipt of the IM-6ARR form and the actions taken. Place the original IM-6ARR in the case record and provide or mail a copy to the participant.
Use guides with instructions for ending an authorized representative in MEDES, are forthcoming.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Beginning January 1, 2014, begin using the new version of the IM-6ARR.
- Updated versions of the forms are available in the Forms Manual on the intranet.
