- MO HEALTHNET FOR AGED, BLIND, AND DISABLED (MHABD) APPLICATION PROCESS FOR CHILDREN LOSING COVERAGE THROUGH THE DIVISION OF DEVELOPMENTAL DISABILITIES
DISCUSSION:
The purpose of this memorandum is to inform staff of the process for MO HealthNet Aged, Blind, and Disabled (MHABD) applications for children who:
- are eligible for services from the Division of Developmental Disabilities (DD),
- are approaching their 18th birthday, and
- are not already receiving MO HealthNet coverage through the Family Support Division (FSD).
The FSD and DD have developed a streamlined MHABD application process for these individuals, to ensure continuity of MO HealthNet coverage after turning 18, if ongoing eligibility exists. These applications may be submitted as early as 90 days prior to the 18th birthday of the individual/applicant.
DD will assist the individual, their guardian or authorized representative, to complete all necessary application forms. The necessary forms along with verification and medical documents will be sent to designated staff located at the Greene County Office for processing.
