DISCUSSION:
This memorandum is to introduce manual revisions due to Email IM-4 dated 01/23/13 regarding state hearing requests.
Effective January 23, 2013 the Hearing Request form (FA560) is no longer mailed out automatically. Instead, the eligibility unit (EU) may request a hearing in person, in writing or by telephone. Provide the EU with an IM-87 Application for State Hearing form when the form is requested. Refer to instructions for the Application for State Hearing.
