DISCUSSION:
This memorandum is to introduce revisions to the form Important Information About Electronic Benefit Transfer (EBT) Transactions (IM-3EBT), IM-3EBT Instructions, and the Misuse of Temporary Assistance Poster. The revisions are due to new laws for public assistance misuse and fraud as introduced in Senate Bill 251, outlined in IM Memorandum IM-#65 dated 07/17/13.
Use the IM-3EBT to provide a signed statement to the Family Support Division from the applicants/participants of Food Stamps, Temporary Assistance, and MO HealthNet acknowledging the following:
- MISUSE OF FOOD STAMP, TEMPORARY ASSISTANCE, AND MO HEALTHNET BENEFITS IS A VIOLATION OF STATE AND FEDERAL LAW.
Provide a copy of the form for all individuals applying for or receiving Food Stamps, Temporary Assistance, or MO HealthNet benefits during the following occasions:
- Face-to-face interview;
- In person EBT card replacement request;
- In person mid-certification review or reinvestigation, and
- When discussing replacement EBT cards due to continued replacement requests.
If contact is not in-person, the information on the form should be reviewed with the individual during the interview. A signed copy does not have to be obtained. Make a comment on the Eligibility Unit Member Role (EUMEMROL/FM3Z) screen stating when the IM-3EBT form is provided to an applicant/recipient or reviewed over the phone.
The form may be placed in the reception area for informational purposes.
FSD offices must display the revised Misuse of Temporary Assistance Poster in their lobby.
