DISCUSSION:

The purpose of this memorandum is to introduce the revised Referral/Information for Child Support Services (CS-201) form. In response to suggestions from Income Maintenance (IM) and Child Support (CS) staff, Child Support Program and Policy streamlined the form while ensuring the form still gathered information necessary to begin providing services. The revised form is much shorter, now two pages instead of four.

The CSE-201 form requests basic information needed to locate, contact, and secure support from the absent parent and is used by Child Support to begin working with custodial parents. It is essential that the form be completed as thoroughly as possible as it provides the basis for the Child Support case.

The CS-201 form and instructions are attached, and are available in the IM Forms Manual Volume I. Use existing forms to depletion. If forms are ordered from the warehouse, staff will continue to receive the prior (4 page) version until those supplies are gone.
