DISCUSSION:
The purpose of this memorandum is to introduce the revised signature request letter and to explain processes when an unsigned application is received with other documentation. Begin using the revised signature request letter immediately.
REVISED SIGNATURE REQUEST LETTER
- The following statement was added to the letter:
All documentation you provided has been returned to you and must be resubmitted along with your signed application.
UNSIGNED APPLICATIONS
- For MAGI categories, follow instructions in Income Maintenance memorandum IM-45 Changes to the Process for Unsigned Applications for MO HealthNet for Families, Women, and Children dated May 6, 2015 for Integrated Tracking and Reporting System (ITrackRS) processes and unsigned application instructions.
- For all other programs, follow instructions in Income Maintenance email memorandum IM-7 Unsigned Applications dated January 26, 2015 for ITrackRS instructions and IM-23 Unsigned Applications and the Signature Request Letter dated March 13, 2014 for instructions on returning all applications to the customer for signature.
- Return documentation provided by the individual with the unsigned application.
- Do not scan documentation provided with an unsigned application.
- Do not scan the unsigned applications
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
