DISCUSSION:
The purpose of this memorandum is to introduce the revised version of the IM-31A SMHB ELECTRONIC form. The language on the attached version reflects the current business process for SMHB.
Discontinue use of the IM-31A ELECTRONIC language found with Memorandum #110.
NOTE: The IM-31A SMHB ELECTRONIC is only intended to request necessary insurance information for SMHB. If income verification is needed, a separate IM-31A ELECTRONIC should be sent to the applicant.
NECESSARY ACTION:
- Disregard the language on the IM-31A Electronic found with Memorandum #110.
- Use the attached IM 31A ELECTRONIC SMHB for requesting insurance information for SMHB applicants.
- If income verification is necessary for SMHB then send a separate IM-31A ELECTRONIC.
