DISCUSSION:
The purpose of this memorandum is to introduce the revised version of the IM-32 SMHB Action Notice dated 02/02/2016. The form has been updated to allow for expanded text in the reason boxes.
Discontinue use of the IM-32 SMHB Action notice with a revision date of 12/20/15 found with Memorandum #110.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Discontinue using the IM-32 SMHB Action notice dated 12/20/15.
- Use the attached IM-32 SMHB Action notice for all current correspondence.
