DISCUSSION:
This memorandum is to introduce the Claim Referral (CR-1) form to field staff. The CR-1 form is from the Program Integrity Unity (PIU) and will be used to determine if a claim should be established on a case. This includes any Income Maintenance case (i.e. Child Care, MO HealthNet, Food Stamps, and Temporary Assistance cases).
Eligibility Specialists (ES) are responsible for completing the CR-1 form and sending it to the PIU. The ES must provide as much information as possible on the CR-1 and all documentation to support establishing a claim. The PIU may contact the ES and request assistance in obtaining further documentation to establish the claim.
The CR-1 form can be found in the Forms Manual as CR-1. Return the completed CR-1 form to the PIU point of contact from the referring county. The PIU Claim Referral Points of Contact information can be found on the intranet on the QA/QC Compliance page.

- Review this memorandum with appropriate staff.
