| Some of the FAMIS IRS 1099 forms mailed to child care providers have incorrect income amounts printed on the forms.
The income amounts were corrected and revised 1099 forms are scheduled to be remailed this week with ‘AMENDED COPY’ printed on the 1099 forms. FAMIS generated IRS 1099 forms show the return address as: Department of Social Services We apologize for the inconvenience. |
