• a FSD staff member speaks with the person, explain the options of applying for Family MO HealthNet programs:
  • Through the Contact Center by calling 855-373-9994
  • Online at https://mydss.mo.gov/
  • By going to the local FSD Office
  • By mail
• a message is left requesting an application, and the address is known, mail out the following by the next business day:
  • Application Request Letter
  • IM-1SSL application
  • IM-4 Finding Help Brochure
