4.7.5 Grievance Process
When a FST meeting participant believes there to be a procedural error or bias which prevented consensus from being reached during a FST meeting:
- The participant may request from the worker a Request for Review of Family Support Team or Adoption Staffing Team Decision form (CD-190). The form must be submitted within 3 business days of the Adoption Staffing team meeting decision or within 5 business days of the Family Support Team meeting.
- Refer to CWM 6.7.8 (Family Support Team Decision Review) for more guidance.
When a parent or youth feels they have been discriminated against by the Department of Social Services:
- The parent or youth may review the Non-Discrimination Policy Statement to determine if their concern applies. If so, they may request from the worker a Client/Applicant Complaint of Discrimination (DSS-CP3).
When a child or adult family member or CD-licensed placement provider has concerns of perceived inequities, unfair treatment, or dissatisfaction with agency actions or behaviors not covered by other grievance procedures:
- The individual may request from the worker a Service Delivery Grievance Form (CS-131).
- Refer to CWM 8.1.1 Service Delivery Grievance Process for more guidance.
- Resource Providers who work with FCCM/contracted case management agencies must utilize the FCCM agency’s grievance procedure. For more information they may contact the FCCM case manager or FCCM licensing worker.
