If an applicant has previously applied for or received assistance from FSD, there should be an existing record. After receiving a new signed application, review the existing pieces of evidence and compare the existing evidence with changes that may have occurred since the case closed.
No further documentation is needed for eligibility factors previously provided or verified. Identity, date of birth, Social Security number, or citizenship that is unchanged should still be available in the applicant’s record.
