If the participant is unable to write their name, they may sign by mark. A mark is generally an X or another symbol. If the signature is by mark, the mark should be identified as such and enclosed in parenthesis with the participant’s name written. If the signature is by mark, two witnesses are necessary. The signature and address of the two witnesses is required.
