Applications for the Family MO HealthNet program must be available and accepted at FQHCs and DSHs. If a FQHC or DSH does not have an eligibility specialist on site, the facility’s staff is allowed to accept applications and forward them to the local FSD office.
These same procedures apply to other facilities that contract with FSD to accept MO HealthNet applications. All contracted facilities are required to send paper applications to FSD within 5 business days.
