APPLICANT'S ELIGIBILITY STATEMENT ADDENDUM (IM-2 ADDENDUM) AND FORM INSTRUCTIONS; FORMS MANUAL REVISION #28
DISCUSSION:
The purpose of this memorandum is to introduce the APPLICANT'S ELIGIBILITY STATEMENT ADDENDUM (IM-2 ADDENDUM).  This form has been developed, to be used in conjunction with food stamp applications taken in the FAMIS system and all IM applications.  To prevent duplication of information required from the participant, all food stamp references have been removed.

Counties will not receive an initial distribution of these forms.  They will need to be ordered from the warehouse as counties come up for FAMIS food stamps.  This form is to be used when the participant's applying for food stamp benefits (in a county that is taking food stamp applications in FAMIS), and an IM application is being taken at the same time.
NECESSARY ACTION:
  • Order the IM-2 Addendum prior to your county coming into FAMIS.
  • Use the IM-2 Addendum when taking a Food Stamp application in FAMIS in conjunction with an IM application.
  • File the IM-2 Addendum and the forms instructions in the Form's Manual.
