PARENTAL NOTICE OF FAMILY CHILD CARE PROVIDER REGISTRATION APPROVAL/REJECTION (IM-94B) FORMS MANUAL REVISION # 1
 
DISCUSSION:
Changes have been made to the Parental Notice of Family Child Care Provider Registration Approval/Rejection, IM-94B.  These changes enable staff to use the form when a provider applies for or renews registration, and when a provider's registration is closed in the interim. Attached is a copy of the revised Parental Notice of Family Child Care Provider Registration Approval/Rejection, IM-94B. 

The title has been revised to read "Parental Notice of Family Child Care Provider Registration Status".

The phrase ".your child care provider has been denied." in the second and third options has been updated to ".your child care provider has been denied/closed."
NECESSARY ACTION:
  • Discuss this memorandum with all appropriate staff.

  • Begin using the revised IM-94B upon depletion of existing supply.

    •  
  • Remove form IM-94B dated 1/00 and instructions dated 3/00 from the Forms Manual. 

    •  
  • File revised form and instructions (Adobe Acrobat) in the Forms Manual.
