DISCUSSION:
The IM-115 REQUEST FOR FOOD STAMP ELIGIBILITY UNIT REPORT is now available in an electronic version.  Food stamp benefit history is stored for three years on FPAY in the Legacy System, and on the Payment History screen in FAMIS. 

If payment history is needed prior to three years, request the benefits history by completing the IM-115 Request for Food Stamp Eligibility Unit Report.  Attach the completed IM-115 to an EMAIL to Cole CoIDM.

This form is only available in electronic version.  To download the form go to the Family Support Division Web site.  Under "Forms", click on "Word Templates".  Follow the instructions for downloading an individual form.

A payment history report (IM-116) is mailed to the county office.
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.
  • Use the electronic IM-115 to request food stamp payment history not displayed in Legacy or FAMIS.
  • File the IM-115 and instructions (pdf file pdf) in the Forms Manual.
