THIRD PARTY RESOURCE FORM (TPL-1) AND INSTRUCTIONS
DISCUSSION:
The Division of Medical Services has revised the Third Party Resource Form (TPL-1).  There has been no change in policy for use of this form.  Revised instructions (pdf) are included with this memorandum and are self-explanatory.

Continue to use the existing TP-1 until depleted.  The new TPL-1 is in supply at the warehouse.
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.

  • File the revised form and instructions in the Forms Manual.
