Effective immediately, form IM-120A, Food Stamp Summary of Performance and instructions is obsolete.  Form IM-209, Case Reading, and monthly reports received by county offices include the information from this form.  Please destroy/recycle this form and instructions.
 
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.
  • Destroy/recycle form IM-120A and instructions.
