SUBJECT:
FORMS MANUAL REVISION #8:  REFERRAL/INFORMATION FOR SERVICES (CSE-201); CASE OPENING NOTIFICATION LETTER TO MEDICAID-ONLY CUSTODIAL PARENT/CUSTODIAN (CSE-206A)
DISCUSSION:
Based on suggestions from child support enforcement staff the Referral/ Information For Services (CSE-201) form has been revised. The revised forms and instructions (pdf files) are included with this memorandum and are self-explanatory.

This change is being made to:

  • Reduce the number of modifications required to add the child support obligation after the Division of Child Support Enforcement has established an order for medical support only; and
  • To more clearly explain the level of services available via the CSE-206A. 
  • The option of choosing medical support only will no longer be made on the CSE-201.  Participants are given the opportunity of making this choice after approval on the CSE-206A.
NECESSARY ACTION:
  • Review this memorandum with appropriate staff. 
  • Print and file the revised CSE-201 and CSE-206A forms and instructions in the Forms Manual. 
  • An initial distribution of this form will arrive in county offices shortly. 
  • Destroy existing forms upon receipt of initial distribution.
