| CHANGE TO FA325 REQUEST FOR INFORMATION |

Effective March 7, 2004,
the FA325 Request for Information is revised to allow staff to add up to
eight lines of text. The text box is at the bottom of the FA325 and
is titled "Additional Instructions are Listed Below."
The new text box allows the worker to give the EU specific information regarding the documentation to provide or the activity to complete before their eligibility can be determined. Text is added to the FA325 one line at a time. Follow instructions outlined in the "Editing and Adding Text to the FA325" user guide. |
