Requests for a supply of MC+ applications from agencies outside of
the Family Support Division should be directed to Martha Bax at 573-751-3216
or via email to
Martha.J.Bax@dss.mo.gov
. Requests for MC+ applications
should NOT be referred to Lane DePrima of the Missouri Care Primary Care
Association.
Refer to IM-99 dated July 30, 2003 for information regarding where to
refer inquiries concerning outreach activities.
