The DFS GED Billing Form has been revised as of March 2004 and is now entitled FSD GED BILLING FORM.  The revised form is electronic only.

The administration fee will continue to be direct billed by Department of Elementary and Secondary Education (DESE) to the Family Support Division (FSD).  The administration fee will be paid for individuals case managed by FSD who are in an educational component in the JOBS system.  The testing center fee is to be paid by the individual taking the GED test directly to the local testing center.  A column for the individual’s birth date has been added to the form next to DCN.

The statement “Attach School Permission with Application for Missouri Certificate of High School Equivalence (GED) when applicable” has been added to the informational box.  (See School Permission box on application to determine when applicable.)

FSD Central Office staff will review and authorize the payment.  To accomplish this, additional fields are added to the bottom of the form.

  • Review this memorandum with appropriate staff.
  • GED administration fees for individuals in an educational component in the JOBS system and case managed by Family Support Division will be paid directly to DESE.
  • Remove the DFS GED Billing Form and instructions from the Forms Manual.
  • File attached FSD GED Billing Form pdf file and instructions pdf file to the Forms Manual and add to the Table of Contents replacing the DFS GED Billing Form.
