FOOD STAMP/TEMPORARY ASSISTANCE INTERVIEW GUIDE FOR HURRICANE KATRINA EVACUEES
DISCUSSION:
Staff who are taking food stamp and/or Temporary Assistance applications and interviewing the EU away from an FSD office may not be able to access FAMIS to complete an interview. An EU may not be able to come to the office, send a representative, or have access to a telephone for the interactive interview.
To aid staff in completing an application interview for food stamps and/or Temporary Assistance, attached to this memorandum is a guide that includes the questions in FAMIS for food stamps/Temporary Assistance programs.
- For food stamp applications, ask the applicant to complete the FS-1, then use the Interview Guide to complete the interview.
- For Temporary Assistance applications, complete an IM-1 and use the Interview Guide to complete the interview.
This guide is to be used until October 15, 2005, along with the food stamp or Temporary Assistance applications for EUs applying for assistance as a result of being evacuated from his/her home state due to Hurricane Katrina. Upon return to the FSD office, enter the information from the interview guide into FAMIS. Attach the interview guide to the food stamp or Temporary Assistance application completed by the EU.
Follow Hurricane Katrina Food Stamp Evacuee Policy issued in Memorandum IM-#100 and EMAIL Memorandums IM-#118, IM-#119, IM-#121, and IM-#122. Follow Income Maintenance Evacuee Policy issued in Memorandum IM-# 104.
