REVISED IM-311 MISSOURI EMPLOYMENT AND TRAINING PROGRAM (METP) REFERRAL AND RESPONSE FORM
DISCUSSION:
The IM-311 Referral and Response form and instructions are revised and renamed to Missouri Employment and Training Program (METP) Referral and Response form. The IM-311 is used to re-refer food stamp EU members who are sanctioned or are being sanctioned to the Division of Workforce Development (DWD) to end a food stamp sanction. The use of the IM-311 for Temporary Assistance was discontinued per Memorandum IM-#71, dated June 30, 2005.
The revised IM-311 is formatted as follows:
- The first section is the “Referred To” section and is used to enter the name and address for the DWD office to which the participant should report for METP participation.
- The second section is the “Individual Referred” section and is used to enter the name, address, telephone number, DCN, Social Security Number, and date of birth of the individual being re-referred to DWD.
- The third section is the “Referred For” section and is used to let DWD know if the individual is being re-referred to prevent a sanction or end a sanction, comments, the caseworker to whom a response should be provided, the caseworker's telephone number, and the fax number to which the completed form should be returned.
- The fourth section is the “Response” section and is used by DWD staff to respond to the FSD worker. DWD will indicate whether the re-referred individual began an activity, refused to participate, or didn't show up. The DWD worker will also provide any comments, their name, phone number, fax number, and date the form is completed.
All other sections formerly on the IM-311 are deleted.
An initial supply of the revised IM-311 will be mailed to county offices. Upon receipt of the revised form, recycle/destroy the old forms.
NECESSARY ACTION:
- Review this memorandum with all appropriate staff.
- Begin using the revised IM-311 upon receipt when re-referring food stamp participants to DWD.
