INSTRUCTIONS FOR IM-31A, REQUEST FOR INFORMATION (SPANISH)
DISCUSSION:
The IM-31A, Request for Information (Spanish) is revised to correct the Division of Family Services name to Family Support Division.
Each item on the IM-31A is numbered to enable staff to match a specific item to the IM-31A instructions for translation.
Use the IM-31A (Spanish) for every food stamp, Temporary Assistance, or Medicaid application or interim contact for EUs who speak Spanish. When using the Spanish translation of the IM-31A, mark the same items as those on the FA-325 Request for Information for FAMIS programs.
The IM-31A (Spanish) and instructions are available on the Intranet. To access the form, go to the Family Support Division Intranet home page and follow these steps:
- Click Food Stamps, Medicaid, or Temporary Assistance;
- Click Forms;
- Click Forms Manual;
- Click Volume 1;
- Scroll down to IM-31A (Spanish) Request for Information in Spanish; and
- Click on either the form
or the instructions .
This form is not being printed and cannot be ordered from the warehouse.
