IM-4(EBT) REVISION
DISCUSSION:
The IM-4(EBT) 
has been revised. The revisions include:
- information about using ebtEDGE,
- updated information about using the card and PIN for cash and food stamp transactions,
- removal of all references to food stamp coupons, and
- frequently asked questions to assist cardholders in using their benefits through EBT.
An initial supply of the IM-4(EBT) will be sent to the county offices. When the initial supply is received, destroy/recycle all versions of the IM-4(EBT) dated prior to revision date 07/05.
The IM-4(EBT) is also available:
- through the Department of Social Services Intranet in the Forms Manual, Volume I

- Review this memorandum with appropriate staff.
