REVISED IM-112 ACTION TAKEN ON YOUR FOOD STAMP CASE
DISCUSSION:
The IM-112 Action Taken on Your Food Stamp Case form is revised. FAMIS generates the FA-150 Action Notice to EUs for actions taken in FAMIS. The IM-112 form is for actions that occur outside of FAMIS. The revisions include adding the following case actions:- rejection of a food stamp application because the application is made prior to 60 days from the end of the current certification (desk rejection);
- approval of a request to replace food stamp benefits;
- denial of a request to replace food stamp benefits;
- approval for a reduction of a food stamp claim balance; and
- denial for a reduction of a food stamp claim balance.
The following information is removed:
- Section A, approval of a food stamp application;
- Section B, certification periods;
- Section C, expedited food stamp benefit information;
- Notice of Expiration of Food Stamp Benefits section;
- Section E, food stamp benefit reduction due to an overpayment:
- Section F, Electronic Benefit Card authorized representative notification;
- Section G, special living arrangement benefit issuance guidelines; and instructions to reapply for food stamp benefits.
The IM-112 Action Taken on Your Food Stamp Case is available on PCs as of 02/10/05. If the IM-112 is not on your PC, follow the instructions below:
- on the Family Support Division Intranet page, click on Food Stamps;
- click on Forms;
- click on Word Templates; and
- follow instructions for download.
