IM-31, APPOINTMENT LETTER/REQUEST FOR CONTACT AND INSTRUCTIONS
DISCUSSION:
The IM-31, Appointment Letter/Request for Contact, is revised to clarify the use of the form.
The form is divided into two sections.
- To schedule an appointment for an interview for an application or reinvestigation, complete the top section, Application/Recertification/Reinvestigation for Continued Eligibility.
EXAMPLE: Mr. D dropped off a food stamp application at the office and left
before a staff member could interview him. He did not list a phone number on his
application. Complete the top portion of the letter scheduling an appointment for
the interview.
- Use the lower section, Request for Contact, to obtain or clarify information received and have been unable to reach the individual by phone.
EXAMPLE: Ms. E applied and was approved for food stamps and is a simplified reporting EU. She left a message for her caseworker that she has obtained a different job and does not know whether or not she should report it because the income has changed.
