DISCUSSION:
Form IM-2D, Reinvestigation - Eligibility Statement for the Adult Programs is revised. A new question added to the income section asks whether the individual or spouse has ever served in the military. If the response is “yes”, pursue eligibility for Veteran's benefits.
Destroy/recycle all previous versions of the IM-2D. An initial distribution has been sent to the county offices. Order the form from the warehouse following normal ordering procedures. The IM-2D, Reinvestigation-Eligibility Statement is also available via Department of Social Services Intranet in the Forms Manual, Volume I. Staff will be notified when the instructions become available.
