CHANGE OF ADDRESS FOR DIVISION OF MEDICAL SERVICES SPENDDOWN AND MC+ CHIP PREMIUM PAYMENTS
DISCUSSION:
The spenddown and CHIP premium payment addresses have changed. The new addresses are listed on the invoices which recipients are receiving. However, some recipients are continuing to use the old addresses and this could result in delays of coverage.
To alert recipients to the change, the Division of Medical Services will send brightly colored notices advising of the change along with all invoices sent out beginning in June. These notices will be sent for three months (June, July, and August). Copies of the spenddown and MC+ CHIP premium notices are attached below. These copies are for staff informational purposes only and should not be sent by Family Support Division staff to recipients.

Please note that each program has its own address. Therefore, when providing the address to recipients who may have lost an invoice, it is extremely important that the correct address be provided, to prevent delays in coverage.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Provide the new address for the appropriate program to recipients who request premium payment addresses.
