DISCUSSION:
Senate Bill 539 (2005) eliminated prescription and office visit co-payments for children eligible for MC+ for Children CHIP Premium levels of care. Forms IM-32PRM (MC+ Approval Notice - Premium) and IM-33MCC (MC+ Notice of Action) have not been updated to reflect this change, due to the conversion of Family Medical Assistance cases to the FAMIS system. When using these forms for cases in the Legacy system, cross out the sentence which states:
You will be responsible for $10 co-payments for each office visit and $9 for each prescription.
