Form IM-103 
, Electronic Benefits Transfer (EBT) Available Date for Food Stamps on the Regular Payroll and instructions are revised to remove reference to the Division of Family Services and replace it with Family Support Division.
An initial shipment of this form is being sent to each county office. Upon receipt of the revised IM-103, destroy/recycle all versions previous to 05/06. Use normal ordering procedures to obtain this form from the warehouse.
This form is also available on the Intranet, in the Forms Manual, Volume II.
NECESSARY ACTION:

Review this memorandum with appropriate staff.
Destroy/recycle all versions of this form and instructions previous to 05/06.
