FORMS MANUAL REVISION #3: IM-23 CLIENT SERVICES POSTCARD AND INSTRUCTIONS

DISCUSSION:

Form IM-23 pdf logo, Client Services Postcard and instructions were recently revised and an initial distribution of the postcard (rev. 10/06) was sent to each county office. However, because of postal restrictions, the initial shipment should not be used. Upon receipt of the revised form (01/07), destroy/recycle all versions with a revision date prior and up to 10/06.
