MEMORANDUM

DISCUSSION:

This memorandum is an update to the MO HealthNet Outreach Project established to provide information to families of uninsured children identified by a Missouri public school or a licenced childcare provider.

Senate Bill 583 of 2010 requires the Family Support Division (FSD) to collaborate with the Department of Elementary and Secondary Education (DESE) in an effort to reach the uninsured children of Missouri.  Refer to Income Maintenance memorandums IM-77 MO HealthNet Outreach Project With Department of Elementary and Secondary Education and Family Support Division and IM-78 Senate Bill 583 and Licensed Childcare Providers  dated December 22, 2010 which explain the initial procedures established for the MO HealthNet Outreach Project.

With the introduction of the single streamlined application (IM-1SSL) for Family MO HealthNet programs it was necessary to update this process.  In May 2014, to comply with Senate Bill 583 the FSD provided the DESE and licensed childcare providers with a flyer that informed parents to apply on-line at https://mydss.mo.gov or to request an application by calling 1-855-FSD-INFO.

NOTE:  Effective immediately, the SB583-IM-1UA will no longer be accepted as a valid application for Family MO HealthNet programs.  All SB583-IM-1UA applications should be destroyed by Missouri public schools and licensed childcare providers.

If a completed SB583-IM-1UA is received, FSD staff should:

• Immediately mail the applicant an IM-1SSL application (if there are more than 2 persons in the household include additional pages of the IM-1ADP).
• Include an IM-31A to inform the applicant the SB583-IM-1UA is no longer a valid application.

NECESSARY ACTION:

• Review this memorandum with appropriate staff.
• Upon receipt of a SB583-IM-1UA, immediately mail the applicant an IM-1SSL and IM-31A.
• Destroy any blank SB583-IM-1UA applications.
