Section 5, Chapter 4 (Case Record Transfer Procedures), Subsection 1 – Family Centered Services (FCS) Case Transfers

There are times during work with a family that a case may need to be transferred. When those situations occur, the following must be completed and documented.  

5.4.1.1 Family Centered Services (FCS) Family Remains in State 

When a family is planning to move or has moved from the county in which the Family Centered Services (FCS) case was opened, the worker shall document and complete the following: 

  • Determine when the family is moving or confirm with the family or collateral contact that the family has moved,  
  • Obtain the new address or location of the family, 
  • Ensure participation in a supervisor consult to: 
    • Evaluate the facts and circumstances of the case to determine what, if any, current safety threats and risk factors are present, and 
    • Determine if the Children’s Division (CD) should continue to work with the family. 
  • If determined during the supervisor consult at CD should continue to work with the family, then the worker must complete and send the Case Transfer Request (CD-175) to the circuit where the family is now located. The worker should provide a brief summary on the CD-175 of the case progression, including the safety goals and current status of the case; and 
  • Record all activities, up to the date of last contact, within seven (7) calendar days. 

The new county of residence shall verify the location of residence and authorize the record to be transferred within ten (10) business days, then update the FCS function in the system of record.  

 

5.4.1.2 Intact Family Moves from State 

 If the family has moved out of Missouri, the worker shall complete and document the following:  

  • Ensure participation in a supervisor consult to evaluate the facts and circumstances of the case to determine what, if any, current safety threats and risk factors are present;  
  • Determine during the supervisor consult safety threats or risk factors are present. If safety or risk factors are present, the worker will contact the child protection services agency in the new state of residence and provide the following information:   
    • A summary of why CD was involved with the family; 
    • A summary of current safety threats and risk factors; and  
  • Complete the closing summary and closure of the FCS function in the system of record  

The family record will not be transferred out of state.
