6.7.4 Guidelines for Informal Meeting Decision Letter to the Resource Provider

Within five (5) days of the informal meeting, the Regional Designee must notify the resource provider of the decision to uphold or reverse the disputed case management decision. The following information must be included in the written decision attached to the CS-70:

  1. The name of the child(ren) who is the subject of the decision;
  2. The date the informal review was conducted;
  3. A summary of the informal review and a description of the material reviewed and persons interviewed;
  4. If the case management decision is reversed, an explanation of what the Division or it’s contractor staff will do to reflect the reversal.
