If the participant has a court appointed conservator, the conservator must complete and sign the application.
If the participant has a court appointed guardian, either the guardian or the participant may sign the application.
Obtain documentation of the legal guardian or conservator. Add information to the eligibility system. Refer to FAMIS Resources and MEDES Resources for guides.
NOTE: If the participant has a court appointed conservator, but signs the application on their own behalf, the application must be considered unsigned. See 1802.020.05 Application With No Signature.
For other situations, send a Request for Interpretation of Policy (IM-14).
