Applicants must be informed about their rights and responsibilities. This is provided in writing in the eligibility determination notice (FA-150) and verbally during the Food Stamp Interview. FSD staff shall explain the policies regarding the following to each applicant prior to beginning the interview:
- option to provide/not provide SSN (see 1105.040.00 Social Security Numbers)
- option to provide/not provide immigration status (see 1105.010.00 Citizenship and Immigrant Status)
- option to not provide consent to contact immigrant’s sponsor (see 1105.010.20 Sponsored Immigrants)
- verification required to establish eligibility (see 1102.000.00 Verification)
- voluntary withdrawal
- how the program will operate for the EU
- right to fair hearing and appeal
- policy regarding quality control
- processing standards and service complaints
- federal and state laws regarding fraud provisions and penalties
- non-discrimination
- service referrals
- EU’s responsibility for reporting changes
- disclosure of information/confidentiality
- procedure for obtaining and use of the Food Stamp benefits via EBT (see 0150.000.00 Electronic Benefit Transfer (EBT) System/Direct Deposit)
