The Supplemental Nutrition Assistance Program (SNAP) application must be signed. The application can be signed by the applicant, a responsible household member, an authorized representative or legal guardian or conservator (acting as authorized representative).
When a legal guardian, conservator, or other individual serves as the authorized representative and completes the application, advise of the need to report required changes on behalf of the household according to 1140.005.05 Change Reported Timely.
Note: SNAP households are liable for over issuances resulting from inaccurate information provided by the household members or authorized representatives.
Valid signatures include written, typed, or electronic.
Individuals who apply through mydss.mo.gov and choose “Apply Online”, are required to attest to the signature, and must agree to the terms and conditions.
Individuals who apply by uploading the SNAP application (FS-1) in a Portable Document Format (PDF), must type their name on the signature line for the application to be valid, and are not required to check an attestation box.
