Form IM-87, Application for State Hearing is revised. The purpose of the revision is to:
At the time the IM-87 is faxed to the Administrative Hearings Unit, complete field 23 to indicate if follow-up documents are being sent separately. When the follow-up documents are actually sent to the Hearings Unit, enter the date in Field 24. Attach a copy of the hearing request with the follow-up documents. This alerts the Hearings Unit that these are additional documents for a previously requested hearing and helps to eliminate a duplicate hearing for the individual. The IM-87 form and instructions are available on the Intranet. An initial shipment of the IM-87 is being sent to county offices. Upon receipt, destroy/recycle all versions of the IM-87 and instructions prior to 10/04. |
