The IM-4FS, “Eat For Health” brochure, is revised. Changes to
the brochure are as follows:

references to Division of Family Services are replaced with Family Support
Division;

the gross income maximums for household sizes 1-8 are updated;
-
the DSS web site is included for individuals who wish to download an application
with instructions for making a food stamp application; and
-
the 800 telephone number was removed.
This brochure may be obtained from the warehouse using normal ordering
procedures. Upon receipt of the revised brochure (11/03), destroy/recycle
all previous versions. File a copy of the revised version in the
IM Forms Manual.
